Appointments

Each clientt will undergo an intake assessment.  The Intake Assessment process is a comprehensive review of every client’s medical, physical and behavioral health history.  This assessment is secondary to the eligibility clearance.

The eligibility clearance is inclusive of assessing each patient’s level of cognitive, level of mobility and payor source eligibility.
